Adoption checklist

  • An employee should provide notice to their line manager and HR no more than seven days after the date on which they are notified of having been matched with a child for the purposes of adoption, or advise the anticipated birth date and intended leave if having a child via surrogacy. if this is not possible, as soon as is reasonably practicable.
  • The employee should state when the child is expected to be placed with them and the date on which they would like their adoption leave to start.
  • The relevant form (Per_32a – Application for Adoption Leave and Pay) will need to be completed and sent to the HR at least 21 days before Adoption Leave/Absence begins (where possible). The Adoption certificate and confirmation of Primary Adopter Status (if applicable) or a parental order will also need to be submitted – Section 2 of attached form PER32a.
  • Upon receipt, HR will confirm in writing the date on which adoption leave will end. An individual may return sooner than this date, providing they give at least 28 days’ notice in writing.

Whilst the dates above are as per policy, we understand these may not fit every case, please ask for support from HR regarding your own case where your timings differ and we will endeavour to support where needed.
